“重建联系”（Reconnect）项目通过社区为基础的早期干预服务，为12至18岁无家可归或面临无家可归风险的青少年及其家庭提供帮助。该项目协助青少年稳定生活状况，并提升他们在家庭、工作、教育、培训及当地社区中的参与度。 “重连”项目的工作人员针对青少年及其整个家庭采取一系列干预措施和策略，以提供及时的干预。该项目提供的服务范围包括：
